Our nurses communicate directly with DSCC care coordinators and physicians to keep plans current.
Does your child qualify?
Nursing care is funded through the MFTD Waiver. Children qualify when they meet the following criteria.
Under 21 years of age
Must be less than 21 at time of enrollment. Adults enrolled before 21 may remain for life.
Medical & technology needs
Child must require skilled nursing due to both medical complexity and technology dependence.
Illinois Medicaid eligible
Child must be enrolled in or eligible for Illinois Medicaid. Parental income is not a barrier.
Illinois resident
Child and family must reside in Illinois. Kaff serves Chicago and surrounding communities.
Medically fragile
Condition requires ongoing skilled nursing supervision that cannot be safely provided without trained staff.
DSCC assessment required
Eligibility is confirmed through DSCC at UIC — they assess the child and develop the care plan.
